Support Built Around You
YR Thrive Support Services provides individualised, mobile NDIS support focused on helping participants build confidence, independence, skills and meaningful community connection. Support can take place in a participant’s home or in suitable community settings such as parks, libraries and other local spaces.
Support is shaped around each participant’s individual goals, interests, strengths and preferences. This may include accessing the community, exploring nature, participating in creative activities, developing everyday skills, trying new experiences or simply building confidence through meaningful participation.
What Support Can Look Like
No two participants are the same, so support doesn't need to look the same either. Activities and experiences are guided by the participant’s goals, interests and individual needs.
Support focused on emotional wellbeing, confidence, recovery, strengths and working towards individual goals.
Building everyday skills, confidence and independence through capacity building, mentoring, coaching and practical skill development.
Supporting participants to access their community, explore interests, participate in social and recreational activities and build meaningful connections.
Creative activities and art-based experiences can be incorporated into support where they align with a participant's interests, goals and preferences. Individual and small-group creative opportunities may also be available.
Supporting communication and interpersonal skills, self-advocacy and navigating services and community supports.
Assistance with daily living skills and participation in everyday activities, including transport to community activities where funded and appropriate.
